CLUB MEMBERSHIP ACTIVATION / E-MANDATE

 

TERMS AND CONDITIONS

By signing and submitting this form you agree to all of the following terms and conditions:

 

AUTHORISATION

By submitting this document you activate your club membership and authorise Storm Combat to collect a monthly fee in accordance with the Club Membership Option selected below. This payment is collected via debit order from the bank account detailed in this form.

 

DATE OF PROCESSING

Fees are payable in advance and debit orders are usually processed on the 1st of each month.   

 

JOINING FEE

Payable once off upon joining the club:

Joining fees are as follows:

Adult Membership : R650

Kids Membership : R450

BEGINNER COURSE CANDIDATES

At the end of the 12 Week Beginner Course, your club membership remains valid and you may continue your journey to more advanced training.  You will have access to all of our classes and your fees will remain the same.  This Debit Order will continue to be processed beyond the termination date of the initial introductory training.  If you do not wish to continue training beyond the 12 Weeks, please confirm in writing one week prior to the end of the course and we will terminate the Debit Order immediately upon your instruction.  Failure to do so will result in the Debit Order continuing until we receive instruction from you to terminate.

CANCELLATION POLICY

Debit Orders can be cancelled at any time, simply by notifying the club. One weeks’ written notice is required.  Debit Orders not cancelled in time will continue to run for the month.  No refunds are granted on debit orders that are deducted due to late notification.  Memberships may be cancelled, but not paused.

REJOINING THE CLUB AFTER A CANCELLED MEMBERSHIP

Should you cancel your membership and wish to re-instate your membership at a later stage, please note that a new joining fee will be charged.  Please bear this in mind if cancelling over December with the intention of re-instating the membership over January. 

 

HOURS OF OPERATION

The club operates according to the hours listed on the timetable. The club is closed on Sundays and Public Holidays, and over Easter and the Christmas season in the latter part of December.  Group classes may be cancelled if less than three students are booked.

 

SCHEDULES

Timetables are refreshed / changed / updated periodically in order to meet changing needs of our members.  

 

GRADING and CLOTHING

It is compulsory to train in a Storm Combat T-Shirt in the club.  You will be required to purchase this from our online store within one week of signing up. Only black shorts or black leggings are permitted.  Grading Exam Entry Fees and Fight Gear (Boxing Gloves, Shin Pads, Mouth Guards, Groin Gaurds, Head Guards, MMA Gloves etc) are not included in monthly membership fees. 

 

FEE INCREASES

While we will always endeavour to keep our fees as affordable as possible, fees increase from time to time. Should a fee increase be scheduled, you will be given advance warning through our WhatsApp Official Communications Group and you will have the option to continue or terminate your debit order.

At COR Concepts (and including this website, POPIAct-Compliance) we are committed to protecting your privacy and to ensure that your personal information is collected and used properly, lawfully and transparently.

We collect and process your personal information to contact you for the purposes of understanding your requirements, and delivering services accordingly.  For this purpose we will collect contact details including your name and organisation.

We will use your personal information only for the purposes for which it was collected and agreed with you.  In addition, where necessary your information may be retained for legal or research purposes.

Your Rights: Access to information

You have the right to request a copy of the personal information we hold about you. To do this, simply contact us at the numbers/addresses as provided on our website and specify what information you require.  We will need a copy of your ID document to confirm your identity before providing details of your personal information.

Please note that any such access request may be subject to a payment of a legally allowable fee.   

 

Correction of your information

You have the right to ask us to update, correct or delete your personal information. We will require a copy of your ID document to confirm your identity before making changes to personal information we may hold about you. We would appreciate it if you would keep your personal information accurate.

 

Definition of personal information

According to the Act ‘‘personal information’’ means information relating to an identifiable, living, natural person, and where it is applicable, an identifiable, existing juristic person.  Further to the POPI Act, COR Concepts also includes the following items as personal information:

  • All addresses including residential, postal and email addresses.​
